1. Press ## followed by the OTKSL2. SVC menu will be displayed
3. Press 1 for Phone #
4. Enter the 10 digit MIN and press OK
5. MIN that was entered will be displayed press OK
6. SVC Menu will display press 3 for NAM
7. Enter the 4 digit Home SID press OK
8. Press END and handset will power cycle
